LIBRARY INSTRUCTION REQUEST FORM

UCSC Instructors may use this form to request a library class. To schedule a library session for upper-division or graduate courses, contact a subject specialist (http://library.ucsc.edu/infonotes/Coll.html). Submit your request at least two weeks prior to your preferred class date.

Note: The Library provides instructional support for lower-division, undergraduate courses Monday-Friday 8 AM-5 PM. For courses scheduled outside these hours, please contact an Instruction Coordinator to see if alternatives can be arranged.
